When you schedule an appointment, that time will be reserved especially for you. We recognize that your time is valuable and strive to minimize your wait. In order to maintain our schedule and minimize wait time, we ask that you keep scheduled dental appointments and provide at least 24 hours notice on cancellations.

Dental Insurance Card
If you have dental insurance, bring your insurance card so that we may make a copy for your record. This will allow us to assist you in processing dental claims and ensure the quickest possible reimbursement.
If your previous dentist has taken x-rays, you may request that they forward them to our office or you can bring them to your appointment with you. If additional films are necessary, they can be taken at our facility. Medical Status Information
A list of medications you are presently taking.
Please alert the office if you have a medical condition that may be of concern prior to performing dental procedures, or a condition that requires pre-medication with antibiotics. The American Heart Association recently updated the list of conditions that require antibiotic pre-medication. Many patients who once took antibiotics prior to dental visits may no longer require pre-medication. Conditions that might require antibiotic pre-medication include uncontrolled diabetes, artificial heart valves, previous joint replacement surgery, pulmonary shunt, congenital heart defect, and previous bacterial endocarditis. Please contact our office with any questions prior to your appointment.
